As the seasons begin to change, conversations about the immune system seem to appear everywhere.
Patients ask me what they should be taking to “boost” their immune system. Advertisements promise stronger immunity through the latest supplement. Social media is filled with advice on preparing for the cold and flu season.
I understand why.
None of us enjoys being sick, and if better health could be purchased in a bottle, life would be much simpler.
Yet over the years, I’ve become less interested in the idea of how to boost the immune system and more interested in a different question.
What does it actually mean to have a healthy immune system?
The answer is far more interesting—and far more complex—than I think most of us realize.
For many years we were taught to think of the immune system primarily as our defense against infection. While that is certainly true, modern immunology has revealed something much more remarkable. The immune system is involved in wound healing, tissue repair, cancer surveillance, metabolic regulation, and constant communication with the nervous system, endocrine system, and the microbial communities that inhabit our bodies. The deeper scientists look, the more interconnected the picture becomes.
Perhaps the most fascinating aspect of the immune system is that so much of its work goes unnoticed. Every meal introduces foreign proteins. Every breath carries particles from the environment. Trillions of microorganisms live peacefully within us. Every day, our immune system is making countless decisions about what requires a response and what should simply be left alone.
That ability to discriminate may be one of its most important functions. Sometimes I think of the immune system less as the body’s army and more as its intelligence network.
An intelligence agency doesn’t exist simply to fight. It gathers information, recognizes patterns, communicates with other departments, remembers previous encounters, determines whether something represents a genuine threat, coordinates an appropriate response, and, just as importantly, knows when the situation has been resolved.
The metaphor isn’t perfect—biology rarely is—but I find it more useful than thinking of the immune system as something that simply needs to be made stronger.
Because a healthy immune system isn’t simply an active one. It’s an appropriately responsive one.
Two immune systems, one conversation
One of the most helpful distinctions I explain to patients is the difference between our innate and adaptive immune systems. The innate immune system is our first responder. It acts quickly. It recognizes broad patterns associated with injury or infection and begins responding almost immediately. It doesn’t need to know exactly what it is encountering to recognize that something may be wrong. The adaptive immune system works differently. It learns. Every significant infection, every meaningful exposure, every vaccine contributes to its education. It develops memory, allowing the body to respond differently the next time it encounters the same challenge.
One system provides speed. The other provides precision. One recognizes patterns. The other remembers experience. Neither functions particularly well without the other.
What fascinates me even more is that neither functions independently of the rest of the body. The immune system is in constant conversation with the nervous system. Hormones influence immune signaling, while immune cells produce signals that influence the brain. The gastrointestinal tract houses a significant portion of our immune tissue, while the microbes that live within us participate in educating and shaping immune responses throughout life.
This is where many of us get lost. We naturally want to ask whether something is a gut problem, a hormone problem, an immune problem, or a neurological problem. The reality is that biology doesn’t tend to organize itself that way. Our medical specialties do, but the body does not.
Capacity before disease
One of the privileges of practicing integrative medicine is that I often meet people before they meet the criteria for a diagnosable disease. Their digestion has become less resilient. Seasonal allergies are becoming more persistent. Recovery from illness seems slower than it once was. Their energy no longer returns after a good night’s sleep. Perhaps their blood sugar has begun to drift upward, or inflammatory markers have quietly changed over time. None of these findings, by themselves, necessarily define a disease. Yet together they often tell a story. The body’s capacity to adapt is beginning to change.
Recently, I reviewed blood work with a patient whose symptoms initially seemed unrelated. She had longstanding allergies, digestive symptoms, and subtle changes in her metabolic markers. Individually, each finding could easily be managed in isolation. Together, however, they suggested something different.
They suggested a physiology that was gradually becoming less adaptable.
Was this a gut issue?
An immune issue?
A hormonal issue?
The most honest answer was probably all of them.
Not because one system was causing the others to fail, but because these systems are continuously influencing one another. The body had not divided itself into specialties, even if medicine often does.
This is where I find Ayurveda offers an interesting perspective.
In the Charaka Samhita, there is a concept known as Vyādhikṣamatva. It is often translated simply as “immunity,” but I think that translation is too narrow. Classical commentators describe it as the capacity to resist the development of disease and the capacity to withstand disease when it does occur.
What I appreciate most about this idea is the question it asks. Not, “How do we prevent illness altogether?” But rather, what gives one person greater capacity to adapt to life’s inevitable challenges than another?
Thousands of years later, I think that’s still one of medicine’s most interesting questions.
The problem with quick fixes
Our brains like simple explanations. If I feel tired, there must be one cause. If I have allergies, there must be one allergen. If my immune system isn’t functioning well, surely there must be one supplement that fixes it. Living systems rarely behave that way. The human body is built upon relationships. Sleep influences immune regulation. Nutrition influences immune cell function. The immune system communicates with the nervous system. Hormones influence inflammation.
The gut shapes immune development. Movement changes immune cell circulation.
Every system is influencing every other system. This complexity can feel frustrating. It would be much easier if there were a single pathway to target.
Instead, health emerges from thousands of small interactions occurring every moment of every day. Perhaps this is also why the foundations of health often seem so unremarkable. Eating nourishing food. Sleeping consistently. Moving regularly. Managing stress. Spending time outdoors. Maintaining meaningful relationships.
None of these makes for particularly compelling marketing. None promises dramatic transformation within seven days. Yet these are the very conditions under which physiology tends to function well.
The role of time
One of the hardest conversations I have with patients has nothing to do with supplements or laboratory testing. It’s about time.
Very few people arrive in my office because of what happened last week. The stressful month. The holiday indulgence. The poor night’s sleep. These experiences matter, but they are rarely the whole story.
More often, they are the moment that reveals a physiology that has quietly been adapting for months or years. The body has extraordinary capacity to compensate. Until it doesn’t. Just as health rarely disappears overnight, it is rarely rebuilt overnight. This can feel disappointing in a culture that values quick results. But I actually find it hopeful.
If our physiology changes gradually, then our daily choices matter far more than we often realize. The quiet work accumulates. A nourishing meal may not transform the immune system. Neither will one good night’s sleep. Nor a single walk outside.
But repeated over weeks, months, and years, these ordinary acts become part of the environment in which our physiology continues to adapt.
The story isn’t particularly dramatic. It doesn’t make headlines. There is no highlight reel for consistency. Yet this is how biology has always worked.
Learning to ask better questions
The more I understand immunology, the less interested I become in simple answers. Not because science has failed to explain the immune system. Quite the opposite.
The deeper we explore human physiology, the more interconnected it becomes. That doesn’t make me cynical. It fills me with a deep sense of curiosity and respect for what we still have to learn.
Perhaps that is why I continue to return to the concept of Vyādhikṣamatva. Not because it explains modern immunology. It doesn’t. But because it reminds me to ask a question that feels just as relevant today as it did centuries ago.
How do we cultivate the capacity to meet life’s inevitable challenges? We will all encounter infection. We will all experience stress. We will all age. None of us will avoid those realities.
Perhaps health is not measured by how successfully we escape them, but by how well our physiology can recognize them, respond appropriately, recover, and continue adapting.
To me, that is a far richer understanding of the immune system than simply asking how to make it stronger.
